- Click on pmujjwalayojana.com.
- Go to the download form on the homepage and click on it.
- After clicking on the download form, the form of PM Ujjwala Yojana will come.
- Now fill your name, e-mail id, phone number and captcha in the form.
- Now click on the button to generate OTP.
- After that download the form.
Submit the form to the nearest LPG gas agency
Now you have to submit this form to your nearest LPG agency. Along with this, you will have to provide documents like Aadhar card, proof of local address, BPL ration card and photo etc. After the document is verified, you will get LPG gas connection.
